These terms apply to the Functional Longevity Assessment at app.ultrevity.com and this website, provided by Ultrevity, LLC (“Ultrevity,” “we”). By using them, you agree to these terms.

Not medical advice

The assessment and its results, comparisons, tips and reports provide general information about functional performance. They are not medical advice, diagnosis or treatment, and they do not create a doctor–patient relationship. Talk with your doctor before starting a new exercise program or if you have questions about your health.

Your safety

The assessment includes physical activities. Only do activities you can do safely. Use a sturdy chair and keep support within reach. Stop immediately if you feel pain, dizziness, shortness of breath or chest discomfort, and seek medical help if needed. You take part at your own risk.

Eligibility

You must be 18 or older to use the assessment.

Results

Results depend on how the tests are done and on the information you enter. Some results, such as endurance, are estimates. Comparisons with others your age are shown only where reference data allow and may not reflect your individual circumstances.

Your account

Keep access to your email secure, since sign-in links are sent there. You are responsible for activity on your account.

Our content

The assessment, its content and PFLM™ Technology are owned by Ultrevity or its licensors. You may use the assessment and your own results for personal use. You may not copy, resell or reverse-engineer the assessment or its methods.

No warranties

The assessment is provided “as is,” without warranties of any kind, to the extent allowed by law.

Limitation of liability

To the extent allowed by law, Ultrevity is not liable for indirect, incidental or consequential damages, or for any injury arising from your participation in the activities.

Changes

We may update these terms and will change the date above when we do. Continued use means you accept the updated terms.
