This policy explains what information Ultrevity, LLC (“Ultrevity,” “we”) collects through the Functional Longevity Assessment at app.ultrevity.com and this website, and how we use it.

What we collect

• Assessment answers and results: age, sex, height, weight, activity questions, and your results from the six tests.
• “Since your last assessment” answers (returning users, optional): how your activity changed between assessments, anything that may have affected your results, and any notes you choose to add.
• Account information (only if you create a free profile): your first name and email address.
• Basic device information: device type, screen size and browser type, used so results can be compared fairly between sessions on similar devices.

You can take the assessment without creating an account. If you do not create a profile, your results are stored without your name or email.

How we use it

• To calculate and show your results, save your history, provide tips and create your report.
• To send sign-in links to your email address.
• To maintain, secure and improve the assessment.
• To analyze de-identified, combined data (without names or email addresses) to learn what changes are linked to better results and to improve the assessment and its methods, including PFLM™ Technology.

We do not sell your information, and we do not use advertising trackers.

Sharing

We use service providers to run the assessment: Supabase (database and sign-in), Vercel (hosting) and Resend (sign-in emails). They process information only to provide these services to us. We may also disclose information if required by law.

When you use “Copy,” “Share” or “Email my results,” you decide where your results go. Information you share with others, including your doctor or an AI assistant, is handled under their own policies.

Your choices

You can ask us to access, correct or delete your information by emailing support@ultrevity.com.

Security and storage

We use reasonable safeguards, including encrypted connections and access controls, to protect your information. No system is perfectly secure.

Not medical care

The assessment provides general information about functional performance. It is not medical advice, diagnosis or treatment.

Age

The assessment is for adults 18 and over. We do not knowingly collect information from anyone under 18.
